JOB DESCRIPTION:

Our Clinical Staff Pharmacists are an integrated part of the multidisciplinary care team, serving as a resource for antimicrobial stewardship, medication safety, and optimized pharmaceutical dosing. They assist with medication histories, monitor patients drug therapies and provide drug information. Clinical Staff Pharmacists provide pharmacy services to patients, nurses, physicians, and other medical staff by interpreting and evaluating the validity and safety of orders and prescriptions. They integrate core clinical pharmacy knowledge, dispensing and order review activities. Individuals will be expected to perform core clinical activities including order verification, pharmacokinetics, IV to PO, therapeutic interchange, and other as required. These pharmacists work closely with support staff, providing direction as needed. In the absence of a leader, a clinical pharmacist may be required to assume the essential responsibilities and perform the duties of a leader.

The inpatient pharmacy operates 24 hours per day, 7 days per week. The majority of our Clinical Staff Pharmacists work variable shifts, all rotate through weekends and holidays. A few, including the Pharmacist Night Lead, work full time night shifts.

Patient populations serviced include oncology, surgery, medicine, cardiology, pediatrics, adult and pediatric critical care, peri-operative, psychology, same day infusions, renal and rehabilitation.

Our Pharmacist Night Lead works onsite as part of a team of two pharmacists during the overnight hours to support inpatient pharmacy services as listed above.

EDUCATION:

Doctor of Pharmacy Degree (preferred) or Bachelor of Science Degree from an ACPE accredited School of Pharmacy required. ASHP accredited Pharmacy Residency or equivalent experience preferred. Licensed Pharmacist in the State of Vermont required.

EXPERIENCE:

Two years clinical pharmacist experience in a health system preferred.

Responsible for overall direction, utilization, supervision, instruction, and evaluation of professional and support staff, and students’ utilization in the Rehabilitation Services Department. Establish policies, procedures, objectives, and work standards while ensuring provision of services in accordance with all state and federal laws and regulatory agencies.

5.      5.Establishes appropriate goals and treatment plan taking into consideration diagnosis, age, cultural, ethnic, and religious beliefs of patient and family. Involved patient and family in determining goals and treatment plan. Provides physical therapy treatment consistent with treatment plan and goals utilizing a variety of techniques. Confers with person(s) most closely associated with active management of the patient. Modifies treatment plan and goals as needed depending on the condition of patient.

 

6.      6.Ensures adequate staffing to provide quality services. Ensures an efficient staff by effectively interviewing candidates, preparing and conducting performance evaluations, verbal counseling, working with Human Resources on any progressive disciplinary action, and making recommendations for promotions as indicated. Addresses a d performance or conditions immediately.

 

7.      7.Ensures training of staff in accordance with department and hospital policies and procedures to provide quality services. Ensures training of staff in code of conduct and HIPAA training.

 

8.      8.Directs department meetings and communicates information to staff in a clear and timely manner. Encourages group problem solving and staff participation in program development and promotion of the department. Participates in facility meetings, teams, and committees as required.

 

9.      9.Attend patient care conferences as needed. Keeps abreast of new techniques and trends in the rehabilitation fields. Attend conferences, meetings, or special courses whenever possible and share with department personnel information gained thereby.

 

10.  10.Assesses need for and develops and implements new programs to meet needs of patients, referral sources, and community. Performs direct and indirect marketing with current and potential referral sources. Participates in community activities to promote good public relations.

Position Summary

Responsible for the management and performance of the Environmental, Health, and Safety for MMS San Diego locations (Mira Mesa and Otay).  Responsible for leading, implementing, and maintaining effective environmental, health, and safety programs.  Ensures a safe work environment free of recognizable hazards.  Ensures compliance with all federal, state and local regulations.

Duties & Responsibilities

·            Responsible for the overall management of all facility environmental health and safety programs including ERT, Safety Representatives, Hazard Communication, Occupational Safety, Ergonomics and Industrial Hygiene.

-         Responsible for both Mira Mesa and Otay locations. Expected engagement 50/50 between sites.

·            Provide supervision, coaching and development for site leaders.

·            Lead projects & evaluate processes in the anticipation, recognition, evaluation and control of occupational health, safety and environmental regulations.

·            Develops and provides safety and environmental training to all site and field associates.

·            Conduct jobsite assessments, focusing on unsafe conditions and unsafe work behaviors; ensure appropriate action is taken to correct deficiencies.

·            Participate in Occupational Safety and Health inspections and in any procedures that may follow a citation. 

·            Investigate, prepare, and forward, as required, reports on injuries and fatalities.

·            Develop safety policies, goals, programs and training.

·            Monitor and maintain records and permits of hazardous, radioactive, and biohazardous materials and waste in compliance with all applicable local, state and federal regulations.

·            Develop and communicate technical information in a manner that fosters understanding, decision-making, and positive interaction with employees, functional managers, & facility management teams.
